Our Membership Solution is not dental insurance and cannot be combined with any insurance plan, other offers, or discounts. By enrolling, you agree to be billed the Total Membership Amount according to your selected RECURRING billing frequency using the card you provide.

Achieving and maintaining excellent oral health is a lifelong commitment, and we hope your membership continues as long as you have teeth!

Minimum Membership Term

Your membership is billed for a minimum of 6 months and cannot be canceled during this period. After the initial 6-month term, cancellation must be requested by the patient.

Non-Transferable Benefits

Membership benefits are non-transferable.

Forfeiture of Unused Services

If treatments or services are not utilized during the eligible time period, they will be forfeited.

Active Membership Requirement

Included services and privileges are only redeemable if your membership is active and payments are current. Membership benefits cannot be used if the membership is suspended.

Appointment Policy

Appointments are reserved especially for you. If you need to reschedule, 48 hours notice is required. Failure to provide proper notice or a “No Show” for a scheduled appointment will result in the forfeiture of the scheduled benefits.
